Because counseling involves a professional relationship, we need to agree on the terms of our work together. The Client Information and Service Agreement forms will need to be completed by all new clients prior to their first appointment. If you choose to use insurance, you will also need to present your photo identification and health insurance card in order to verify your insurance benefits. If your sessions are to be conducted via telehealth, please also complete the Consent to Participate in Telehealth form. The Release of Information form is to be completed only by request.
